What ACA Marketplace Health Plans Cover (and How Private Plans Compare)
What does an ACA marketplace health plan actually cover?
Every ACA marketplace plan has to cover ten categories of “essential health benefits” — doctor visits, hospital and emergency care, prescription drugs, mental health and substance-use treatment, maternity and newborn care, rehabilitation, lab work, pediatric services, and a long list of preventive care at no extra cost. It also can’t turn you down or charge you more for a pre-existing condition. Private under-65 plans are built differently: what they cover varies plan to plan, they can be medically underwritten, and some cover less — which is often exactly why they cost a healthy person less. Knowing precisely what each option covers for your situation is what a broker is for. Ryan Michalek at Michalek Health Solutions, an independent broker in Tampa, Florida (licensed in 31 states), will walk you through both at no cost.
When people compare health plans, they stare at the premium. But the premium only tells you what you pay — not what you get. The more useful question is what’s actually covered, and here the two paths differ sharply. ACA marketplace plans cover a federally defined set of benefits that is the same floor on every plan. Private under-65 plans are flexible and often cheaper, but what they include is up to the plan. Here’s a plain-English, benefit-by-benefit look at what a marketplace plan covers — and where a private plan may differ — so you can choose with your eyes open.
The ten essential health benefits
By law, every ACA marketplace plan — at every metal tier — covers ten categories of care, called the essential health benefits. They are the same floor whether you buy Bronze or Gold:
- Outpatient care (the doctor visits you get without being admitted to a hospital)
- Emergency services
- Hospitalization, including surgery and overnight stays
- Pregnancy, maternity, and newborn care
- Mental health and substance-use disorder treatment
- Prescription drugs
- Rehabilitative and habilitative services and devices
- Laboratory services
- Preventive and wellness services and chronic-disease management
- Pediatric services, including dental and vision care for children
A private under-65 plan is not required to cover this list. Many cover a great deal of it; some leave out categories like maternity, or limit prescription or mental-health coverage. That is not a knock on private plans — it is the trade-off that lets them cost less for someone who does not need everything on the list. It just means you have to read what a given plan actually includes before you count on it.
Free preventive care — at no cost to you
This is the benefit people most often do not realize they already have. On a marketplace plan, a defined set of preventive services is covered at no cost to you — no copay, no coinsurance, nothing toward your deductible — when you use an in-network provider, even before you have met that deductible:
- Adults: screenings for blood pressure, cholesterol, diabetes, and many cancers; routine immunizations; and counseling for things like diet and quitting tobacco.
- Women: well-woman visits, mammograms and cervical-cancer screening, prenatal care, birth control, and breastfeeding support.
- Children: well-child visits, the full immunization schedule, and vision and developmental screenings.
Private plans vary here too — some include strong preventive coverage, others handle it differently — so if routine, no-cost preventive care matters to your family, it is worth confirming on any plan before you enroll.
Pre-existing conditions are always covered
On an ACA marketplace plan, a pre-existing condition — anything from asthma or diabetes to a past cancer diagnosis or a pregnancy already underway — cannot be used to deny you coverage, charge you more, or carve that condition out of your plan. There are no health questions and no waiting period for it. Private under-65 plans work differently: many are medically underwritten, meaning the insurer can look at your health history and decline, rate up, or exclude a condition. If you have significant ongoing medical needs, that difference usually points toward the marketplace — and it is the first thing I check when we talk.
Mental health and substance-use care
Marketplace plans cover mental health and substance-use treatment as one of the essential health benefits, and federal parity rules mean those benefits cannot be more restrictive than the plan’s medical and surgical coverage — so therapy, counseling, and treatment are covered much like a visit for a physical condition. This is an area where private plans differ the most: some cover behavioral health well, others limit it. If this coverage matters to you, it belongs at the top of the comparison, not buried in the fine print.
Maternity, newborn care, and breastfeeding support
Every marketplace plan covers maternity and newborn care — and it is covered even if you are already pregnant when you enroll, because pregnancy cannot be treated as a pre-existing condition. Marketplace plans also cover breastfeeding support and counseling, plus the cost of a breast pump, at no extra charge. Maternity is one of the benefits private plans most often limit or exclude, so for anyone planning a family it is a decisive thing to check plan by plan.
Birth control and dental
Birth control: marketplace plans cover FDA-approved contraceptive methods and the related counseling at no cost to you when prescribed — it is part of the preventive-care benefit. Dental: this one trips people up. Dental coverage for children is an essential health benefit, so it is included in marketplace coverage; dental for adults is not, and is usually bought as a separate standalone plan. Private medical plans generally work the same way on adult dental. If dental matters to you, we will plan for it rather than assume it is included.
| Benefit | ACA Marketplace plan | Private under‑65 plan |
|---|---|---|
| Essential health benefits | All ten, on every plan | Varies — not required to cover the full list |
| Pre‑existing conditions | Always covered, no extra charge | May be underwritten, rated, or excluded |
| Free preventive care | Defined set at no cost in‑network | Varies by plan |
| Mental health & substance use | Covered, with federal parity | Varies — sometimes limited |
| Maternity & newborn | Always covered, even if already pregnant | Often limited or excluded |
| Typical best fit | Ongoing conditions, maternity, or subsidy‑eligible | Healthy, over the subsidy range, wants a lower premium |
So how do private under-65 plans compare?
None of this makes private coverage “worse.” It makes it different. A private under-65 plan is not bound to the essential-health-benefits list, so it can be leaner and — for a healthy person who earns too much to get a marketplace subsidy — meaningfully cheaper, often with a broad PPO-style network and year-round enrollment. The honest trade-off is that “leaner” means you have to know what is in and what is out: some plans are medically underwritten, and some cover less than the marketplace floor. The right choice depends entirely on your health, your income, and what you actually need covered — which is the whole point of comparing them side by side instead of guessing.
How I help
I read the actual coverage — marketplace and private alike — against your real situation, tell you in plain English what each one does and does not cover, and help you pick the plan that fits. If the marketplace is the better home for your needs, I will say so, even though it is not the product I place. There is no fee to work with me and no pressure to enroll.

What’s covered — straight answers.
Does marketplace health insurance cover preventive care for free?
Yes. Every ACA marketplace plan covers a defined set of preventive services — screenings, routine immunizations, well visits, and more — at no cost to you when you use an in-network provider, even before you meet your deductible. The list covers adults, women, and children. Private plans vary, so it is worth confirming preventive coverage on any non-marketplace plan.
Are pre-existing conditions covered on a marketplace plan?
Yes. Marketplace plans cannot deny you, charge you more, or exclude care because of a pre-existing condition, and there are no health questions or waiting periods. Private under-65 plans are often medically underwritten, so they can consider health history — which is why someone with ongoing conditions is often better served by the marketplace.
Does a marketplace plan cover mental health and therapy?
Yes. Mental health and substance-use treatment is one of the ten essential health benefits, and federal parity rules keep those benefits from being more restrictive than medical coverage. Behavioral health is one of the areas private plans differ on most, so check it carefully if it matters to you.
Does marketplace insurance cover maternity if I am already pregnant?
Yes. Maternity and newborn care is an essential health benefit, and because pregnancy cannot be treated as a pre-existing condition, you are covered even if you are already pregnant when you enroll. Maternity is a benefit many private plans limit or exclude, so it is a key thing to compare.
Is birth control covered at no cost?
On marketplace plans, FDA-approved contraceptive methods and the related counseling are covered at no cost to you when prescribed, as part of the preventive-care benefit. Coverage on non-marketplace plans can differ, so confirm it on any private plan.
Does a marketplace plan include dental coverage?
Dental care for children is an essential health benefit and is included in marketplace coverage. Adult dental is not an essential health benefit and is usually bought as a separate standalone plan. Most private medical plans handle adult dental the same way.
What are the ten essential health benefits?
They are outpatient care, emergency services, hospitalization, maternity and newborn care, mental health and substance-use treatment, prescription drugs, rehabilitative and habilitative services, lab services, preventive and wellness services, and pediatric services including children’s dental and vision. Every marketplace plan covers all ten; private plans are not required to.
Do private under-65 plans cover the same things as marketplace plans?
Not necessarily. Private plans are not bound to the essential-health-benefits list, so what they cover varies — some come close to a marketplace plan, others are leaner and cost less for a healthy person. The only way to know is to read the specific plan, which is exactly what I do with you.
How do I know exactly what a plan would cover for me?
Ask before you enroll, and compare the actual coverage rather than the premium. As an independent broker, I go through what each option — marketplace or private — does and does not cover for your situation, at no cost to you, so there are no surprises later.
